Grand Fisher

The Hollow that killed Masaki Kurosaki: for fifty-four years it has evaded Soul Reapers by luring victims with a girl-shaped lure, visible only to spiritually aware humans.

Ichigo faces it at the cemetery, on the anniversary of his mother's death, and wounds it without managing to kill it. Grand Fisher returns years later as an Arrancar, sure he can finally take revenge on Karin: Isshin, his powers restored, cuts him in two.
